Town of Lunenburg issued a check under your name/company that has been inactive for a year or more and it indicates that this check(s) was never cashed and is now considered void.

Please provide evidence that you are the claimant. The payee(s) of the unclaimed check must sign the form. If the payee of the unclaimed check is deceased, please provide evidence that the claimant(s) is the executor of the estate.
If all evidence requested is not received, this claim will not be paid.
According to the statute, checks are presumed abandoned unless claimed by the person entitled to them within one year after the date prescribed for payment.
